Initial Assessment
The initial home care visit by the home infusion nurse typically
lasts up to two hours. This visit includes a complete clinical
assessment of the patient, including all other medications the
patient is taking, the safety of the home environment, and the
ability of the patient and/or family to learn self-administration.
Information gathered in the assessment is communicated to the
physician and used to develop a multi-disciplinary plan of care.
This care plan includes medication clinical pathways and is individually
tailored to each patient on an ongoing basis depending on the response
to drug therapy.
The patient care team provides continuous support, monitors therapy
compliance, and inspires confidence in learning self-care.
