guide.
The Vantage® PPO makes every effort to ensure that this
participating provider network listing is accurate. However, provider
information does change regularly. It is the ultimate responsibility of the
patient to verify with the provider that the provider is participating in the
Vantage® PPO.
Please note that not every provider in a group practice may be participating and that some office locations of a particular provider may not be participating. Patients are encouraged to be specific when verifying provider participation. And, when patients are referred to providers for additional medical services like x-ray procedures and laboratory tests, they should confirm that these providers are in the Vantage® PPO.
Some patients may be covered by a health plan that accesses a certain portion of the Vantage® PPO. It is also the patient's responsibility to either verify with their human resource department, their insurer, their plan administrator, or the Vantage® PPO that the provider is participating in their specific health plan.
It is suggested that at the time of a patient's appointment, the patient present his/her health benefit ID card to the provider office staff. The staff can copy it for their file and identify the Vantage® PPO network through which the patient is accessing their service.
Patients are also responsible to know that health benefit plans differ greatly, and that the individual needs to understand the covered services under their specific plan. Not all treatments and services may be covered and the patient should verify this prior to receiving care by contacting their human resource department, their insurer, their plan administrator, or by consulting the most current benefit documentation.
The Vantage® PPO will not be liable for any charges that are not covered as a result of receiving care from a provider listed in this site.
The Vantage® PPO is a network of healthcare providers that have agreed to accept competitive market-based reimbursement for services offered to patients presenting a health benefit ID card bearing the Vantage® PPO logo. The Vantage® PPO as an entity does not process, adjudicate, or pay healthcare claims. For these types of services, or questions regarding these types of services, the employer, patient, and/or provider may contact the insurer or plan administrator as noted on the patient's health benefit ID card.
