Originally formed in 1989 and re-created in 2000, Vantage PPO is a comprehensive regional network of physicians, hospitals and ancillary providers. These physicians and providers agree to provide their services in an affordable manner in conjunction with local health benefit plans.
1985 - Vantage Healthcare Network creates the Horizon Health Plan PPO
1993 - Horizon Health Plan PPO is renamed the Vantage Managed Care Network PPO
1995
- Over 2,000 physicians and other health care providers
- 34 hospitals
- Over 150 ancillary care providers
1995 - Alliance Health Network of Erie, PA, acquires the PPO and in 1996, renames it the Alliance Health PPO.
1999 - Providers and former employers request that Vantage once again create its own PPO
2000 - Vantage PPO re-created

- Over 6,000 physicians and other health care providers
- 38 Hospitals
- Over 200 ancillary care providers
